I wonder if any one can help with the following work related issues.

I was put in the unfortunate position of having to leave my job following my breast cancer dx. I was working for a Lottery Funded Project with only 3 employees. I asked to go part-time but was essentially told no.

Having now finished my ‘active treatment’ I do now have to think about getting another job. I am really uncertain (and wary given what happend with my last employer) about disclosing my diagnosis. If I were to, at which point do I mention it?
On the application form?
At Interview?
On being offered the job?
I can’t seem to find any information on this issue following a cursory glance on Google.
I also have a recon scheduled for August. How do I disclose this also.

I think I saw somewhere a thing about work and BC - in the column on the left ‘Emplyment, travel and financial issues’ - I havent read it for a while but I think there was something there about when to say if you are looking for work. If not then it was somewhere online I read it.
