Second WLE effective treatment?

Hi there,

I have a second diagnosis of DCIS, pr8, er8, her2-. Same cancer, same breast, 4 years apart. Had WLE and radiotherapy last time (no hormone therapy as was told reoccurrence rate would only drop 2%)

Need to decide WLE again or mastectomy. Surgeon says there is evidence that WLE is effective as treatment again but the only studies I can find demonstrate that this is only true if there is also another targeted radiotherapy treatment. (Which I don’t want and wasn’t offered).

Can you give me any links for the study/studies he is referring to please?

Thanks for your post.

It’s understandable that you are wanting to know about the evidence relating to having another WLE (wide local excision) having had a WLE and radiotherapy for a previous DCIS in the same breast 4 years ago.

Treatment decisions are made on a case-by-case basis by your multidisciplinary team (MDT) after considering the benefits and risks of treatment and other factors such as ones general health and any preexisting medical conditions. As you say the studies you have found looked at outcomes following WLE and radiotherapy. It is not usual practice for radiotherapy to be given a second time which may be why this has not been mentioned now.

We are unsure which specific study your surgeon was referring to regarding the option of a second WLE for you, so we would suggest that you ask them to give you details of this to help you in deciding about surgery. If you have not already done so, we would also suggest that you contact your breast care nurse to discuss your options.
