ALP over 500

Hello all

I was diagnosed last June, 17mm IDC grade 3, no nodes, LVI. Lumpectomy,
and Radio. Started Tamoxifen, then change to Zoladex and Exemestane, with Zoledronic Acid. All going relatively well. A few minor issues, but on the whole felt I was having a better “experience” than some others.

I’m in a bit of shock now, and anxiety is taking over. Blood test before Zoledronic acid flagged ALP 330 and ALT 150. Repeat blood test 4 weeks later ALP 550 and ALT 99.

Letter for CT scan with contrast on door mat when I got home from short break for 2 weeks on Sunday, and Onco the following Tuesday.

I’ve got no symptoms, no jaundice, urine normal, no pains. My mind is going towards bone mets, and I am scared.

Anyone had similar and it was not mets? If so, what was causing your raised levels?

Hi I had similar diagnosis and treatment but lobular 2cm. March 2024.
I’ve had dizziness , now a lot better but I think Anastrazole related . Brain MRI was clear .
Did you have a DEXA ? As part of dizziness checks , I had bloods and my ALP is above range .
I’m being retested in a few weeks .
I think it can be related to bone loss . My DEXA showed slight osteopenia , surprised as I have lifted weights for years .
So, it could be related to bone turnover .

Not had Dexa scan. I was told at an earlier appointment that it wasn’t normal practice since I was having Zoledronic acid infusion. The first blood test that flagged the issue was the day before infusion. The second test was about 4 weeks after infusion, and still rising. So I don’t know about the bone density thing.

I phoned hospital after first test to ask if I should do anything, Onco sent appointment and said “it was very fortunate I made contact when I did”.

And now, counting hours to CT scan and thinking the worse. Thought I got past that phase.

So sorry you’ve come back from a much needed break to worry and anguish . I was diagnosed in July of last year with HER2 positive breast cancer and had a mastectomy and chemotherapy and continue right now with her within therapy . The worry of spread or recurrence is always there for me so I’m not surprised you’ve spiralled with anxiety .
Breast cancer can feel like the unwanted gift that keeps on giving !

Sadly any thing out of the ordinary now can cause us to jump to what we feel is the obvious conclusion … but don’t forget that it could be a host of other things causing the raised levels too x

Cancer isnt the only thing that can cause raised ALP … there are so many things that can cause it , the CT scan is to rule out cancer being the cause , it doesn’t mean that this is definitely the reason and certainly doesn’t mean that it is mets causing the raised levels .Is it possible for you to chat to one of the breast nurses at your unit if not you can contact the breast cancer now nurses

I’m very pleased to report my CT scan came back clear of any mets, after several weeks of worry and anxiety.

It did detect gallstones, which I had eliminated as a possibility as I had no symptoms. Also a thickened esophagus. Both need further investigation, probably a camera down the throat (yuk) and possibly gallbladder removing.

So it’s good news on one hand, but more waiting and tests on the other.
