Liver met ablation

Just had a liver met query that I wanted to ask:
So 7 months was dx with mets in hip. Took a while to get in right meds. Then a scan picked it up in spine & liver (1.3 cm).
On letrozole / Denus/ Ribo. And Tumour markers are down ( from 189 to 62).

So doc thinks things are working - but found myself reading about liver ablation etc.
seems there are a few treatments to get rid of it ( and the liver is regenerative). Freezing/ alcohol injection. That work if it is in one spot.
Has anyone gone down that route?
Or should I just let Ribo do its thing?

Yes, I had microwave ablation. I needed to have it twice (tumour regrew after the first time) but am now in remission. I didn’t have mets anywhere else.

Thanks for sharing. Thats a shame it grew back. I do have mets in my hip and some small ones in my spine.

And it seems like the Ribociclib is working.

So just not sure whether to leave the liver and hope it disappears via Ribo or try ablation.?!
May have to speak to my doc when next in.

Definitely worth asking the question. I don’t know whether it can only be offered when the liver is the only site, as it was for me.

Hi, i am so glad i read your post because i have liver metts and i asked for surgery or freezing. My dr was not at all receptive to that idea. She said oh its not usally done. I have 1 very small mett in my liver right on the edge so i really dont see what the problem is. But now i have read your post it actually gives me ammo to go back and say i want it done. So thank you for posting that.
I want this cancer out of me and if i have to freeze it i will. Can i ask if you asked for the procedure or did the doctors offer it to you ?

It was offered to me both times.

I had what was thought to be early stage TNBC. Lumpectomy followed by chemo. Part way through chemo, I had a scan for a different problem and it picked up a liver met. At that point I continued the chemo, had a PET-CT and MRI which confirmed I had no other disease, and had the ablation. Unfortunately I had a fast growing and aggressive cancer. 3 months on, a repeat scan showed the tumour had regrown and that I had a second one.

I then went on immunotherapy (Pembrolizumab) combined with more chemo. About 5 months into that, I suffered an immunotherapy related ‘adverse event’ where my immune system attacked healthy organs. I suffered significant kidney, thyroid and lung damage, and had to stop treatment. But it had significantly reduced the 2 liver mets. 3 months later they were still stable and my health had improved. So the decision was taken to ablate again. Not really too many other options with TNBC and at that point kidneys too damaged to take Trodelvy, which would be my next, and probably last, line of treatment. Thankfully my first repeat scan was clear, so I hope this holds. The interventional radiologist plans to repeat the ablation if it recurs again.

Microwave ablation is performed in a radiology day case unit, I am guessing not all hospitals have them. I don’t think it’s usual to offer liver resection for breast cancer secondaries because it’s a major operation, and as I found first time around, there are no guarantees it won’t just pop up elsewhere in the liver. I am not sure I would have been offered it if I wasn’t triple negative as there are obviously more treatment options for other types of BC.
