Ribociclib - how do they define high risk?

Noted this drug is now licenced for stage 2 and 3 HR+ HER- patients at “high risk of recurrence”. How is this defined- I believed all HR+ cancers had chance of recurrence?

I don’t know, but I’ve been wondering if this is something I could ask to go on. I was put on biphosphonates to reduce risk of recurrence but they caused eye problems in me. This might be a good alternative. I’m sure a nurse will tell is soon, but i think the cancer is high risk if it is larger in size, multi-focal or multi centric, in the nodes, grade 2 or 3. There may be genetic factors too. I’ll ask when i see my oncologist in August.

I wondered that too - I was multifocal with LVI and 2/3 positive nodes.

The press release from Novartis sums it up well. Basically same criteria as the Natalee clinical trial. I have copied it below:

The recommendation is based on results from the Phase III NATALEE trial, which included 5,101 pre-/postmenopausal women and men with HR+/HER2- breast cancer with any lymph node involvement (excluding microscopic nodal involvement), or if no nodal involvement either tumour size >5 cm, or tumour size 2-5 cm with either grade 2 (and high genomic risk or Ki67 ≥20%) or grade 3

Thank you. I had ki67 of 30, hr +, 4 tumours of 19, 18, 9 and 7mm, plus 3 lymph nodes, all idc grade 2. I will ask about this at next appointment.

NICE has now approved the use of ribociclib with an aromatase inhibitor for the adjuvant treatment of HR-positive, HER2-negative early breast cancer at high risk of recurrence.

Risk of a breast cancer recurrence is determined by many factors and this varies for each individual. You may find it helpful to speak to your breast care nurse (if you have one) or treatment team about your individual risk. They can advise if this treatment would be appropriate for you.
