Zoledronic Acid Therapy

Hello.

I am a 77 years old male who is fairly active and play bowls.
In September 2024 I discovered a lump in my right chest.

It was diagnosed as cancer.

The lump was removed in November 2024 together with 15 lymph nodes. My cancer was classified as stage 2.
The lump was 32mm and 3 of the lymph nodes were cancerous.
Since then I have been taking preventative therapies and drugs.

Chemotherapy. Radiotherapy plus Tamoxifen hormon tablets once a day for the next ten years

Now the Oncologist wishes me to take a course of Zoledronic Acid to help prevent cancer getting into my bones. This was never mentioned previously

This is where I am worried and concerned.

I have been asked to see a dentist as you need healthy teeth and gums for the treatment as there is a slight chance of jaw disfigurement from the treatment.

At the moment I feel I am going to refuse the treatment.

The reason is, besides my age, even without the treatment, for no apparent reason two teeth have already fallen out this year and another is loose. I am in no pain from my mouth. I also have for the past couple of years had constant pins and needles in two left hand fingers.
(I mention this as it could be one of the side effects from Zoledronic Acid treatment)

So having teeth and pins & needles issues before I start, what will happen when I start the treatment.

This really is a cry of help and advice.

What are the risks if i take the treatment or refuse?
What are my chances of Bone Cancer? I cannot find any answers.

Hi Tumblewood,
I had 4 6 monthly zoladronic acid infusion and had very little problem after each session. Some people possibly have side effects but not everyone, we’re all different, for me I was ok. Hope this helps withyour decision, if it were me I’d have them.

My Mum had the treatment at 81, she has her last one in November. She has had no problems at all. She has had regular dental checks, but she did that anyway. I had BC years ago and it was not offered then, I wish it had been. Best wishes.

Thank you for your post. It’s understandable you want to know what the risks are if you decide to have zolendronic acid or not and want to know what your chances are of the breast cancer possibly spreading to the bones.

Zoledronic acid is a drug given to reduce the risk of breast cancer coming back and spreading to the bones and other parts of the body. You may be offered zoledronic acid if you have had treatment for invasive breast cancer that has spread to the lymph nodes under the arm.

In rare situations, it may cause some of the jawbone to lose its blood supply and die. This is called osteonecrosis of the jaw (ONJ). You mention you have lost a couple of teeth recently and you have been asked to have a dental check-up. This will be in case you need dental work before starting treatment. If you decide to start treatment, your treatment team and dentist will tell you symptoms to be aware of and may give you a dental alert card to show a dentist if you develop any symptoms, while on treatment. They will also talk to you about any ways you maintain good dental hygiene.

As @clio18 and @edp say, zoledronic acid can be well tolerated, and jaw problems are a rare side effect. However, it is normal to feel concerned about this, especially when you have recently lost 2 teeth. Your treatment team always aim for the benefits of any treatment to outweigh the side effects. I wonder if you have spoken to your treatment team or dentist about your concerns. Your treatment team can talk to you about the possible risk of recurrence and benefits of treatment in your individual situation.

I also wonder if your treatment team are aware of the pins and needles sensation you are experiencing in your 2 left hand fingers. Occasionally, nerve changes causing either increased sensitivity, numbness, tingling (pins and needles) can be a side effect of this treatment, so it may help to talk about your concerns about this too. This can occur if the drug has lowered your calcium levels too much. If this happens, your treatment team can usually treat this with supplements.
